Milton Adult Day Services, a service of Center for Hospice Care, is a safe place for adults/seniors who need health, social, and support services during the day. This community-based service was designed to meet the individual needs adults and seniors who need assistance during the day. Milton Adult Day Services promises to be respectful and offer opportunities for an enhanced quality of life.

We have time set aside for social hour, current events, group, and individual activities (crafts, baking club, billiards, etc.) and much more! Our day also includes exercise, games and a light breakfast, a hot lunch, and a snack in the afternoon. Plus, monthly visits from the Potawatomi Zoo and different entertainers.

To enroll in our program, an enrollment interview needs to be scheduled with our director, this meeting usually takes around 45 minutes. This meeting is designed to review the program, tour the facility, and review all the admission paperwork. We also conduct a quick assessment to ensure that our services are appropriate for the prospective guest.

Before a guest begins services with us, we need to have a physician’s order form, completed by the primary care doctor, testing for tuberculosis (results included), and the enrollment paperwork completed. Information and forms will be provided during the enrollment interview and are also available on our website.

Attendance is flexible and determined according to the needs of guests and family. Scheduled attendance should consist of at least two half days per a week. This not only develops a schedule for your loved one, but it will enable the guest to establish a relationship with the staff and other guests.

We prepare a care plan for each guest which may include monitoring vital signs and health conditions and administering medications. Our RN is a liaison between the guest/caregiver and the physician or other health professionals when needed. We also aid with activities of daily living as described in Milton Adult Day Services admission criteria.
